What is the name of your state? FL. Am I correct that form SSA-1696 is what needs to be filled out in order for my 27 year old son to designate me as his Representative in his SSI case?

Am I also correct that this form then allows me to conduct any business, contact, interact with, and attend appointments with SSA on his behalf and without him present.

His severe anxiety prohibits him ffrom phone use or anything but minimal social contact and he has received notice for SSI meeting on 10/23 to present records of income, cash, support etc., back to June 2002. I want to utilize this form so that I can appear on his behalf at the meeting. He recently won a favorable decision after a 6 year fight - though they amended the date of disability to June 2002.

I called SSA and asked about the form and they said use Form 821. Uh, Form 821 is a work report form. Lawyer said no form is used - only a Power of Attorney. You can see why I need some help. TIA

Unless you can prove him incompetent or unable to handle his financial Affairs , He WILL have to appear with the required documents .

How did He get through the Process of the Application for benefits ?

You can always go with him , and explain to the Personnel there with the Documents .
