I am looking to appeal a judge's decision & order in a small claims matter. I need to know proper procedures and forms which need to be filed with the court(New York). The deadline for filing my appeal is June 29th!.

Any help would be appreciated.

And what did they say when you asked the court staff on how to file an appeal and what documents are required??? You did ask them didn't you???

If, after asking, you still have no idea what to do, go by a local library or bookstore and try to get "How to win in Small Claims in New York". It includes a chapter on appeals and processes.

And, finally, since you seem unable to do a simple internet search, I did one for your and found the following:
"The New York State Unified Court System has a comprehensive brochure, A Guide to Small Claims Court, that explains the process and how to collect a judgment. This publication is available free at the Clerk's Office in each Small Claims Court."
