What is the name of your state? Florida
When I began to work for Wal-Mart I signed on for the Temp insurance they offer called STARBRIDGE. All the information given to me clearly explained the benefits but when I got my policy I called them to explain it to me over the phone. In any of the pamplets given to me at work was there any mention of deductibles etc. It allows up to $2000 per accident and all expenses are refunded. I filled out both forms, one for the medicine ($52) the other for the actual doctor visit. ($342). I got my first reimbursement check from them a week ago for $1.44 for the medicine less deductibles they never mentioned. I called and the young on the phone claimed to be clueless as to the claim for the doctor's visit. But marked my case to be investigated. I sent Starbridge this letter:
I have heard nothing from them as of today and would like any comments any legal eagles out there have. I am not an Attorney but I think this has Class Action potential. If they have done this to be, given Wal-Mart's size, they have done this to others.

To Whom It May Concern:
Several weeks ago I submitted a claim for a doctor bill in the amount of $345 and prescriptions in the amount of $52.88. I also included all the required documentation and receipts. After joining Starbridge I called you and was given a full explanation of the benefits. At no time was there any ever mention of deductibles. I never would have signed for such a useless medical plan had this been revealed. Days ago I received a check in the amount of $1.44 for what appears to be for the 2 prescriptions. I called after receiving this check to inquire about the doctor bill and the young woman was clueless as to this claim and said she had no record of it, only the prescriptions, and would note the case to be investigated.. Let me be totally clear. I will accept only full reimbursement for both claims as this is what I have been told in the literature I obtained from Wal-Mart and verbally from Starbridge I am entitled to, up to $2000 per injury or accident. Failure to do so within 30 days of the date of this letter will result in the following:
• Filing a formal complaint with your local Arizona BBB
• Filing a formal complaint with the State of Arizona & Florida State Insurance Commissioners
• Notifying the powers that be at Walmart about the nature of this so called insurance
• Seeking any & all legal remedies in court for which Starbridge or its parent company will be held liable for any court & legal fees.
I wish any future communication on this matter to be done via mail.
Sincerely,
I have heard nothing from them as of today and would like any comments any legal eagles out there have. I am not an Attorney but I think this has Class Action potential. If they have done this to be, given Wal-Mart's size, they have done this to others.
