What is the name of your state (only U.S. law)? California

I am a US Citizen who recently got married this year to my non-resident resident wife who entered US with a tourist visa and overstayed. We are in the process of submitting forms I-130(Petition for Alien Relative) and I-485(Adjustment of Status). My questions are:
a.) Is my Certification as a US Citizen enough to prove that I am a Citizen of the United States or do i need to pass all other documents such as my US Passport to prove it;

b.) In filling in both forms, what last name my wife should use, her family name when she was still single or my family name since we are already married;

c.) I will use my sister who lives in San Francisco and my wifes' sister and her husband, who lives in Sacramento to help me for additional support because my YTD is only $7,000 and my annual income for 2007 is $22,000. Are they both gonna use form I-864? Right now we live with my wifes' sister and her husband in Sacramento.

d.)Should my wife use I-485 or I-485A coz she had an ITIN number and filed for 2006 tax return, somebody advised her to apply for ITIN although she didn't worked at all. She only received $2,000 as gifts for Christmas and Birthday. She didn't file for 2007,when she knew that she shouldn't file at all

Sorry if i have a lot of questions,we really want to file the petition as soon as possible. Please advice and Thank You in Advance.
