I do not have an arbitration clause; Even if i did, I did not receive a coverage determination to arbitrate. I have a major medical/fee for service Plan, not an HMO. This is just a situation where they are not even willing to admit they RECEIVED the claim!

My suggestion is to write to the regulator of the Bule Cross Plan in your state, with copies of the prior correspondence as an exhibit. And send a copy to the President and General Counsel of the Blue Cross Plan as well.

Quite frankly, I have grave doubts that "camp" would be covered as it is not medical treatment and/or it is likely not an approved provider.
