What is the name of your state (only U.S. law)? NY

The facts:
Started working for ALC (A large company) in 1987.
Started contributing to 401(k) in 1988, exact month unknown.
Married 4/4/1998 - Value of 401(k) unknown.
Divorce filed 2/3/2009 - Value of 401(k) known....lets say $100,000.

QDRO filled out and mailed to Fidelity. Was rejected and confirmed over the phone that it will take a supoena to get either the value on 4/4/1998 or the exact month I started contributing. This is because Fidelity took over management of the 401(k) in 2008.

Is this supoena something I can do myself? Can someone point me to a draft? Fidelity did give me the address to send the supoena to, I just think if I can do this myself, it would save a lot of money.

The alternative is to work with my ex-wife and agree to using 6/4/1988 as the start date, (my first raise, and if I remember right, when I started contributing) and using the Majuska formula.
