sworn affidavit

What is the name of your state? colorado
what is a sworn affidavit ?is it a form i can order?or is it just a notarized document that i can type up myself? will i need these items before i file my case or do i just need them to use as evidence to make my case?

i looked for this form in the forms index but could not find one titled sworn .Would i use a general form?

if anyone knows what type i need for denver colorado it would be appreciated
