taking children out of state

What is the name of your state? Minnesota My ex has moved out of state and has taken our youngest son (9) yrs old and has left our two older sons (16 & 14) yrs old with me so they can finish school. We were never married we were together for approx 13 yrs she has moved to wisconson about 150 miles away I have never given any type of official OK to move my son out of state. We have been to court the end result is we have 50/50 legal custody and she has phyiscal custody I have liberal visitation which is agreeable to both parties. Since court approx 5 yrs or so ago the visitation has allways been my sons are here everyday after school and with me every other weekend (the only change we ever had between us is they stayed with me starting on Fri insted of Sat on every other weekend). During summer my sons would be with me during the day have dinner and spend the night at moms house.
My Question is can she move my son's out of state without my permission (she moved approx 150 miles away) Also I have asked her for her address to her new residence and the name address and phone number to the new school and she tells me she has not recieved this information yet inorder to give it to me! What are my rights Help?What is the name of your state?

What does your paperwork say? Did she file a notice of relocation with the court?

The paper work only states I have Liberal Visitation we share 50/50 Legal custody and She has phisical Custody. The Visitation is as the two parents agree upon. Since the Begining (approx 5 and 1/2 yrs ago) the Visitation has been like this : School year: children go to school then my house after school then I take them to her house after she finished work and drop them off they ate supper then stayed the night then back to school the next day. I would have them every other weekend from saterday morning untill monday evening after school. (My sons started coming on friday nights when I started working days; they would come after school on friday and stay untill monday evening.) Summer time : (3 sons ages 16 yrs old, 15 yrs old as of june13th, also a 9 yr old.) I would pick up my sons daily and have them untill she finished work on the weekdays and everyother weekend I would pick them-up saturday morning and have then untill monday evening when she finished work approx 5:30 pm. My decree has no stipulations of restrictions or anything against me I have been a outstanding father and will continue to be so ! I hope this helps you better understand the situation as I am lost as far as what are my rights. Concidering the great amount of time I have and my sons have grown acustom to.
