What is the name of your state? South Dakota

I'd like to get a national website of registered childcare providers built. 30 states have downloadable marketing lists available for free or a small charge.

Some providers in question have required registration in their respective state. Some states have voluntary registration with the state.

I'm simply going to aggregate the data and sort by state, city, and Zipcode. Only the following will be listed.

<name>
<address>
<city><State><ZipCode>
<phone number>

in addition, if the data is provided by the state, I'll include the following:
License type
License expiration
Capacity.

Any issue with invasion of privacy, slander, libel, etc? if I pull this data together in a nationwide, searchable format?

Daycare Providers are also able to post their own information, even if they weren't imported from the state lists. Those ads will contain as much information as the provider wishes.

Any issues with me doing this from a legal/privacy standpoint? Does Freedom of Information Act come into play as the providers in question are required to be state registered?

Thanks in advance!

I see no problems with what you are intending to do. This is all public information. If the daycare providers provide their own information, however, you would need to check the veracity.
