iwas recently rear ended by a cement truck in nc and deemed a total loss. the insurance company refuses to pay on the claim until the title is signed over by me, but the title was never changed into my name. the loaner offered to sign the title over initially, but no longer is willing to because the insurance company threatened them with tax fraud. the title was never notarized correctly and was sent to me instead of the auto dealer doing all of the paper work. if the title is transfered to me then i have to pay an extensive amount of money on a car that i will never drive. how do i handle this?

All you have to do is give the title to the insurance company. They will have you sign a release of interest and then you get paid. The title does not have to be in your name. The insurance company needs it to dispose of whats left of your car. It will most likely end up in a wrecking yard so it does not matter if the title is correct or not. It will not cost you anything either.
