Please specify whether
1. Business name can be entered as owner of mark for state registered business (US) - Single Member LLC
2. Can "Limited Liability Company" option be selected for entity type for single member llc?
If not, how to fill those fields for single member llc?

What form exactly is it that you are completing? And if it is not a form of the U.S. Patent and Trademark Office (USPTO) but instead a state government form, which state? The details may make a difference.

However, a LLC certainly can own a trademark, including a single member LLC. While for federal tax purposes (and a lot of states have similar tax rules) a single member LLC is disregarded, the LLC is very much treated as a separate entity, much like a corporation, when it comes to pretty much everything else other than tax law.

Because the answer to this question is requested of all posters to the forum: What is the name of your state?

A trademark applicant can be an individual or a partnership or a corporation or an organization or ... the list goes on.

If you are applying for a trademark for your LLC as a representative of your LLC, you will want to include the name under which your business is incorporated and the name of the state where your business is organized.

Now trying to trademark for brand. While filling TEAS Plus trademark application form at USPTO,
1. Initially first question is owner of mark. Will it be Name of person and then check DBA and enter business name. But since business is already registered and it is not assumed business name. Hence can owner of mark be filled as "BusinessName LLC"?
2. For second question entity type, can "Limited Liability Company" be chosen (or) "Individual" (or) "Sole Proprietorship"?

If not, how to fill those fields for Single Member LLC company?

For members of this forum who are not attorneys (the majority of members), it would be the unlawful practice of law and for those members of the forum who are attorneys, it would violate their state Rules.

So, for information specific to your application, you will need to seek help from an attorney in your area or you can find help through the USPTO. The TEAS help screens should be providing you with some guidance. (https://www.uspto.gov/teas/index.html)

With that said, I can tell you that a business name can be entered as the owner of the mark under Applicant Information. Your entity type is what your business is registered as in your state.

For example, if your business is registered with your state as ABC, LLC, that can be the name entered as owner. If ABC is an LLC, you would put in the Legal Entity Information Type: LLC.
