Transfer Land To My Name

My mother lives in the state of Michigan, she is 64, she will soon be admitted into a nursing home. I am in the process of obtaining Power of Attorney Financial and Power of Attorney Health Care over her. She is divorced but owns land in Abbeville County, South Carolina with her ex-husband/ my father. Both of their names appear on the deed. I want to know how I can have my name added to the deed so when she passes her half will be passed on to me.
