Vaccinations and AOS cover page

What all vaccinations are required? If a person had all of the required vaccinations for school will that be enough for the medical exam?

And another question... What does the cover letter of an AOS package supposed to look like... are they picky as to a certain format or are they just wanting all the info and for it to be organized????

And do you write like an explanation or short paragraph about your situation like as a cover "letter"?

When he goes for the medical examination, he should have his record of immunizations with him so the doctor can see what has been done and to determine what, if anything, is further needed.

There is no need to write any type of cover letter unless you want to. They don't read them as they are not relevant.

Agreed about both items.. if there is something you MUST make sure is noted in your case, make an attachment to a form, not a cover letter. Use a 'cover letter' mainly for your own benefit, as a checklist of sorts. It may or may not make it into the permanent file (reports go both ways).
