My grandfather has his house in his name and has become very sick. If he goes into a nursing home and passes away will the nursing take the house if the house is still in his name?

I am trying to talk to him about what he should do since he does not trust everyone.

I don't know the answer to your question. However, I am guessing that yes the nursing home will want their money and will go after his house in order to get paid.

The correct and non-guessing answer is as long as grandfather has the cash to pay for the nursing home, the nursing home will not be a creditor to his estate when he passes. If grandfather owes the nursing home money, the nursing home will file a claim against his estate. Should the executor of grandfather's estate require to sell the house to pay grandfather's final bills and creditors, that is what will happen. If grandfather's nursing home bill becomes delinquent while he is still alive, that is entirely another matter.
