What is the name of your state (only U.S. law)? florida

My 12 year old brother in law has just received his I-20 from private school in Florida and now will be going to embassy in warsaw for his interview. Besides bringing his I-20 with him to the interview, what other materials must he bring? Does he need to filll out a visa application to take with him to the interview? If so, what would he fill out as it is aF-1 student visa and is there a charge and when does he pay. Of course his parents will be going with him to the interview. Thanks

He needs to bring as much evidence as he can gather that he has strong and significant ties to Poland and that he does not intend to immigrate to the US but that he intends to return to Poland upon completion of his studies.
