What is the name of your state (only U.S. law)? OH

I am wondering if there is an associated liability for an employer offering voluntary (100% employee paid) benefits?

For instance, if there is a guaranteed issue offering and it is not communicated to the employee--would the employer be on the hook for a possible death claim should the employee die?

Should employees be made to either sign on or waive benefit offerings?

Please direct me to where I can read more about the employer's responsibility to convey benefit information to their employees.
