What is the name of your state? Arizona

My wife and I are happily married with a one year old daughter. I have a grown, married daughter (age 27) in Texas who we are close to.

I want to make sure my grown daughter gets custody of my baby daughter if both my wife and I pass. My grown daughter wants that too.

Its very unlikely both me and my wife would die, but it still occurred to me maybe I should do something just in case. Should I?

You can make the request and recommendation in both your wills, but it will be up to the probate judge to approve it,

You and your wife can name your adult daughter as a standby guardian in case something happens to you or you are both incapacitated. However, you should be aware that a judge is not completely bound to your wishes if the judge believes another relative would be a better guardian. Most judges are going to respect the wishes of the parents, but they are not required to do so.

I can't seem to find a form by that name, but I made one a simple one, for both of us to sign and put in the folders with our wills. Will this be enough?

Here is the content of the form I made:

STANDBY GUARDIANSHIP
We, the two signers below, Daddy and Mommy, are the parents of BabyGirl, born x/xx/2020.
If both of us are deceased or disabled, we appoint AdultGirl, the adult daughter of Daddy, to be the guardian of BabyGirl.

I reported your post due to the personally identifying information you shared. Here is a revised version:

For something this important, it would be wise to seek the advice of an attorney.
