What can be done when insruance company does not follow through on stipulation?

What is the name of your state? Florida

It has taken a year and a half, but the work comp ins signed a stipulation in March to provide within 30 days a consult with an otrhopedic spinal surgeon and pay past due indemnity benefits. Two months later they finally paid part of my past due indemnity but will not provide the orthopedic surgeon. They have not responded to repeated requests by my attorney to fulfill the stipulation.

What can be done, legally, to punish them and can I seek any type of monetary relief for their "breaking the law" and not providing me proper medical treatment, treatment that was prescribed by their authorized treating physician?

A formal grievance needs to be filed/petitioned with the Judge of your local Division of Workers' Compensation. You cannot sue them for monetary relief over this. Like that idea though, I would have made a fortune just on that issue alone. :D
