What forms do I file to get custody?

My son just got placed in my care through Juvenile Placemen Department. His father who has full custody gets out in October. I WAS advised that father can take him once he gets out. How and what forms do I need to file custody while my son is in the care of juvenile placement agency.
Is it considered child abandonment since the father got 4 YR sentence leaving the kids and in prison

Why did the father have FULL custody of your son prior to the father's incarceration? In other words, why did you not have/share custody?
