What is the name of your state (only U.S. law)? OH

Hello everyone,
I will be driving 12 people on a bus I have (personal bus) for a pick nick and I want them to sign/fill out some sort of a form that in case of accident I should not be responsible for injuries/damages and etc.
Can someone suggest what is the name of the form and where I can find one?
