When does a protection order go unserved?

What is the name of your state? Nebraska

A month ago I was granted a protection order against an individual who has been threatening me and my family for the better part of a year.

Protection orders are typically served by the local sheriff's department, yet these orders have not been served for the past four weeks. The respondent's address is known.

Could you please educate me as to the other reasons or circumstances that would cause a protection order to be either delayed or simply not served?

It could be a matter of just not being home when they are serving papers. The order is not in effect until those papers are served. But I would keep my copy with me at all times. Have you gone to a hearing yet or was there an Ex Parte order granted?
