What is the name of your state (only U.S. law)? Rhode Island

Witness affidavit. Bottom says "Name of PLAINTIFF or DEFENDANT" and then a signature line. Can they write in WITNESS?

Does my witness write one of those names in and the sign their signature or do they write in the word WITNESS and then sign it?

The witness affidavit should have signature lines that says WITNESS (printed name) and WITNESS (signature) beneath it.

The standard witness affidavit has a line where the witness fills in the name of the plaintiff/defendant for which the witness is testifying. Usually the word plaintiff or defendant will be underlined or circled.
