I live in Pa. I was picked up for DUI three weeks ago. I am going to try for the ARD program. I was picked up for underage dui 21 years ago. Will this affect my chances of getting on this program? Also, what questions will the judge ask to qualify me for this program? Thanks...

ARD is at the discretion of the prosecutor, a 21-year old conviction isn't an absolute bar and I suspect most DAs will still consider ARD for you in this case.
You should however consult with a local lawyer. Some of the DAs I've run into up there are real SOBs.
