stepparent has automatic rights to make decisions over natural parent?

What is the name of your state? Vt If a parent has full legal and sole custody can they give permission for new spouse to sign as guardian/parent and leave out information of natural parent? Also, can a stepparent make legal decisions with the permission of the custodial parent without informing non custodial parent of decisions made medically or educationally?? thanks
