Childcare Tax..Refusal

I sent my son to a childcare provider who was not registered. I asked for her ID so I can claim everything in which I have paid to her. This was discussed before hand and now she does not want to. She stated she has already filled her taxes and will not give me her info. This will make a BIG difference in my tax return. What can I do. Is there a amendment form to fill out? Someone told me to fill out the form 2441 and leave the part blank where it asks for the providers ID (ihave her, name, address, phone number) then at the end of the page where it is blank write that she refused to give me her information. Is this correct? Or is there other/or a different form I need?
 


To be honest...I was a first time mom who has never taken her child to someone to watch so I just did not think about it. I figured since we talked about it , it would be fine to get when the time comes.

Actually, it would have been totally inappropriate of you to ask for that information before taking your child to the provider. Tax ID numbers, whether they are socials or EIN numbers are highly confidential and no one would have provided it to you before they took you on as a client....nor should they have done so. Tons of tax fraud can be committed by someone getting that kind of information in advance.

You can file a paper return if you choose to do so, and leave off the taxpayer ID for the daycare provider, and enclose a note explaining the situation. However that is your only option, you can't file electronically and include the child care credit.
