What is the name of your state (only U.S. law)? florida

Hi, I am a legal permanent resident, but I am moving and I need to change my address (moving within US) I need to fill put a form that I daowload from the USCIS website but there are 2 questions that I need same advised with:
1> PORT OF ENTRE TO US?
2> DATE OF ENTRY INTO US?


what is the port of entry? I do not remember? and the date of entry is the date on my I-95 correct?

And the date of that.
