My daughter was a straight A student. Since she has been in her dad's custody her grades have been dropping. They have gone from all A's to A's and B's and currently C's and B's with 1 D on her progress report.

Is that enough of a drop to constitute a COC? Before anyone asks, yes, I communicate with her teacher and the principal.

She also was finally taken to the dentist and has 4 cavities in her permanent teeth (she is 9, and had 0 cavaties). Her dad also denies medical treatment. She was sick last month, and he told me, the NCP, to take her to the doctor. I did, and dad refused to allow her to have the medication and the test that the doctor ordered. She is okay, but it could have just as well went the other way.

I don't want the judge to view this as a frivilous lawsuit, but I want my daughter to be taken care of like she should be.

She is in 3rd grade. The teacher said that it was sudden (it was) and that she is not staying focused or concentrating in class. She has missing homework assignments, but NOT on my visitation time. I check all work, help her study for exams, and work with ehr on spelling projects, and reports whenever possible.

Dad does not want her to take any medication. He doesn't believe in allergies, and he thinks that it is fine for a child to have Strep Throat without treatment.

The grades started slipping in November. They really went downhill after Christmas Break.

Our daughter had an enlarged heart from a virus when she was little (3-4yo) She recently went to the doctor (I took her with dad's consent) and the Doctor wanted to do a Strep Test. They Swabbed her for the test, and her dad went to the clinic and told them NOT to do the test.

It was all covered by my husband's medical insurance. Dad has no job or insurance.

Since she had previously been under the care of a Cardiologist it is important for her to be checked. Her Grandma had Rheumatic Fever as a child, and almost dies several times since from com[plications. I have a family history of heart disease. Some may say its only Strep, but with our daughter it's not.
