What is the name of your state (only U.S. law)? CA I am trying to create the new order and could use a little advice.
FL-341 does not fit the ordered custody and visitation for my case.
Dad has every Sunday not "weekend".
Dad has every Tuesday evening
not one weekday to another.

(4) Other
There is not enough room. Which form should I use for an additional page or attachment?
Also FL-341(c) Holiday Schedule Does not give the space to explain what was ordered.
Additional page?????? or is there another form you know of that I could use?

The simplest way to handle it is to not use the form at all. Use it as a template to create your own "order".
