Can a Waiver of Liability be included as part of a websites Terms and Conditions? Or do they need to be shown separately from one another?

If they can be shown on the same page, is it sufficient to have the website user tick a check box saying that s/he agrees to the terms prior to purchasing a product or service for the waiver of liability to take effect?

Lastly, is there a specific format in which the Waiver of Liability should appear within the Terms and Conditions?
