What is the name of your state (only U.S. law)? Tennessee

I recently (Nov1st) filed for child support and my ex wife filed (Nov 20th) for a change of cuatody.
In our divorce papers it states that mediation must be tried before and legal papers can be filed for a change in custody/visitation. She never even attempted mediation. Also it seems she has filed simply to avoid paying child support or at least trying not to.
I am wanting to file a motion to quash on her case she filed but am having a very hard time finding either the form for it or a good example to use to do this myself. Does anyone have any suggestions where I can go to get state specific forms for this.
