What is the name of your state (only U.S. law)? Illinois

I have supervised visitation with my son, my ex mother-in-law was pointed to supervise them, she has had bypass surgery and can't supervise them at this point. I just now talked to the judge and he said I have to file a motion to either strike it out or name another person has visitation supervisor, I would like to strike it out but if it came down to it I would name another supervisor. Was wondering how I would go about this? I am from Illinois. He also said I can find the correct form on https://www.illinoislegalaid.org?

This case all started when my 2nd ex wife and I split up, and she made a case of false statements against me. Then my first ex wife tagged teamed with her and made my sons vistations supervised (My first ex wife is the one who has the kid). Well since then my 2nd ex wife and I got back together and all the OPs with me and her has been dropped. Does my first ex wife still even have a case against me about our son?

Yes. She does.

For goodness sake!! Why on earth would you get back with a woman that lied about you in court?

We have already been divorced for many years.
I think that Blue's recommendation was for your 2nd wife - but you may already be divorced from her, too.

Is the supervised visitation based upon a court order?

Please check your local county's court website for self-help information. If you can't figure out the steps to take, then you should seriously consider speaking to an attorney.

Yes it is court ordered. Judge told me to fill out the form on www.illinoislegalaid.org, I just don't know which form I need.

For a non-attorney to tell you which specific form to fill out would be considered the unlawful practice of law. You will want to seek assistance from a local attorney.
