You can file a complaint with Licensing and Regulatory Affairs (LARA), Bureau of Community and Health Systems.

How old are you? The next time you have a complaint against the facility, document it in writing so that they can not later say that you never told them anything.

Another immediate need is for you to get a different wheelchair. Call a medical supply company and find out what it would cost to get a wheelchair and where you could get financial assistance to pay for it.

You didn't suffer any injuries so I agree with Zigner, that won't count as a case but you could still file a complaint.
