Yes, I did properly withdraw and this was corrected on my transcript prior to the appeal process. My point is that in my letter of appeal there was a large portion explaining why I took a withdrawal from the course. Because the decision letter incorrectly stated I got an F, it led me to question whether or not my letter was even read or considered.
Does it matter why you withdrew? Because it seems like the school based their decision on their policies, and that your reasons wouldn't change that.

I filed an appeal based on "information no previously known". I asked them to consider the fact that I had to withdraw based on me unexpectedly taking custody of my newborn neice. It seems to me that the decision was already made without even considering my appeal letter since the decision letter had incorrect information.

I don't think the reason matters. It was explained in the policies that withdrawing counts as an attempt, so you should have been fully aware of it.
