Closeing down a S Corp

What is the name of your state (only U.S. law)? PA.
I have been struggling to keep my business afloat in this economy and to make matters worse I tore my biceps tendon which now leaves me unable to work for many months. My question is what does it take to close the doors on my business? Is there a certain procedure to follow? Forms to fill out? Does it take a lawyer?
Any help would be very much appreciated
