i think the OP stated the husband hasn't been home for the past 12 years. they seperated, just never got a divorce. legally, she's married. but been living single for 12 years.
Then that's even more bizarre.

1. This husband signed the birth certificate for someone else's child before he was even married to the mother.

2. After the marriage, he signed two more birth certificates for children that weren't his (biologically) even though he had nothing to do with the mother at that time?

i don't think he signed the birth certificates. i think the hospital just added his name since mom and husband were legally married.

at least that is what i understood for the last two. still unsure about the 1st one since she said they were together at the time.

I guess that makes a little more sense.

In any event, everyone seems to be agreed that she needs to see an attorney ASAP.

I have already stated I have not had much contact with him in 12 years. He has not lived in the house either. He will agree to whatever. We seperated because he was doing drugs and for about 10 of those 12 years he did not have a permanent residense. He has since then got clean, job, place to stay. My issue is not with him, I want to know is it acceptable to the court for us to NOT have visitation and child support arrangements. I want to do it this way, incase something were to happen to me, I don't want him to have custody.

We were togther, when I had my first son. We thought we were doing the right thing. He was a good person, until he got on drugs. My other two children took his name because we were still legally married. The hospital told me that was the way it was.
YOU NEED TO FILE FOR DIVORCE WITH CHILDREN and in that filing you file to disestablish paternity. You have to serve your husband and he will have a right to disestablish paternity. It is not acceptable to forever waive child support and they will want the children accounted for and taken care of. Your opinion does NOT matter. Have you ever received STATE AID?

Also, getting divorced with him having no custody or visitation won't matter if something were to happen to you. Even if he has no custody, he could get custody in the event of your death because he would be their only legal parent. Unless his parental rights are terminated, he would still be legally dad and his rights would outweigh those of ANYONE else in terms of custody.
