IMO? Remove bolded verbiage. Rather, the ending would read something along the lines of..."...<snip>..sanctions, at the court's discrtion, therefore resolving the issues with joint legal decision making & notification with the Petitioner."

Leave out, entirely the change in physical custody- it's contradictory, your opening line says you have no desire for a change in physical custody.
Yeah, I realized that after I posted it. I thought I'd said "Up to and including a change in physical placement" not "physical custody". Perhaps there's not a difference anyway.

Is it possible to ask that his wife be barred from taking the children to the doctor AT ALL? The judge said she shouldn't, but it never made it into an order. Should I get the transcript from that hearing and include it?

pay for the copy of the transcript ...highlight & file that portion WITH your request (prayer) for the court's relief. make sure you also file the records/envelopes with "Mom" and then stepmom's name written in parenthesis, etc. etc.

There IS always a difference. ;)

So I file all of this together, including my 'evidence' with the emphasized stuff highlighted.

That means that I also serve stupidhead with everything at once, yes?

For service... is it better to serve him via his attorney, or have him served at home via private process server?

If YOU have an attorney~ Give it all to him/her & let THEM file it....If you have no attorney & asshat does~ Serve his attorney...if he no longer has an attorney AND you have no attorney~ Serve Him per rules of proper service in your state.

I'm not using an attorney this time.
I don't know if he has an attorney or not. If I call the attorney he's used thus far, are they likely to tell me if they're representing him?

And it's not asshat. It's stupidhead. Asshat is the babydaddy. :p

Sorry, I've confused asshat w/stupidhead (but, alas, the monikers make more sense now!:p )
