What is the name of your state (only U.S. law)? Pennsylvania

Can someone please provide me a proper form of clause for my web hosting company's TOS? Basically, something to explain that should a signed contract end up in legal matters, the legal matters must be settled in the state of PA where "My Company Inc" resides.

Thank you.

You haven't asked for advice. You've asked for actual legal work, which no one is likely to provide to you free of charge. I suggest you contact an attorney for proper wording.
