In Indiana, you would create two documents. One would be a Motion (or Petition) for Final Hearing. The other would be an Order for Final Hearing on which the court would sign and give you a date for a final hearing.
The words Motion and Petition are interchangeable in your paperwork.
The parties are you and your soon-to-be ex.
Relief sought merely means that you need to state to the court what you want.
In this case, it's for the court to set a final hearing....
