What is the name of your state? NY

Mom and Dad want a simple heath insurance modification, both parties agree. If Mom prepares the papers can they both sign it, get it notorized and file it with the court? Or will Dad have to appear and swear before a judge?

Would someone at the Clerk of Court be able to answer this?

Found the answer. She has to add an affidavit to the motion for both of them that they agree to the proposed modification and it's inclusion into the current order.
