What is the name of your state? Ohio

What happens when the insured (drunk driver) her insurance puts in writing they will pay 100% medical bills and they go over the policy limits.

They only have to pay up to the policy limits. Even if it's in writing, it applies only to the damages up to the policy limits. The only way this wouldn't apply is if it said explicitly they would pay for any amount above the policy limits too.
