I have been in the field of education for 19 years and have never heard of such a thing. For a 504 plan to be developed, temporary or longterm all that is required is a note from a doctor stating a disability that somehow imapcts upon educational performance. It is the responsibility of the school district to develope this plan. Usually, a note from a doctor dismissing a child from PE on a temproary basis does not require a 504 and is just put into place. This frequently happens for children with broken bones and seasonal asthma, etc... Children who are able sit on the sidelines to learn rules and may help with scores or what ever is in their ability at the time. But they do not fail. First start with the building principal then if no satisfaction is gained move on the superintendent.
