What is the name of your state? New Jersey

I 've invented a new product, and have already had several prototypes made. It has structure and design and infinite graphics designs.

Do I need a Utility Patent (pending) or a Provisional Patent (pending)?

I am also claiming 'small entity' status (I am low income/disabled).
Do I need to fill out a uspto form for that as well?

Well, if you want a patent, at some point you will need to file a nonprovisional utility patent. Read here: https://forum.freeadvice.com/showthread.php?t=239469

Do I need to file a 'Small Entity' Form? I am a private, disabled citizen.

And, so far, for the Utility Patent Application, I have Forms 01A, 05, 16 & 17.

You don't need a form to claim small entity status, but your claim for small entity status must be in writing. The link above lists all that is required. There is no form, or particular format, that you need to use -- any signed writing that complies with the info in the link will do.
