1.) How long have you worked for employer A? How long have you worked for employer B? Answer ONLY as follows: A - x months/years: B - x months/years.

2.) How many employees work at the exact same physical address as you do for employer A? For employer B? Answer ONLY as follows - A - xx employees; B - xx employees

3a.) In the last 12 months, did you work 1,250 hours or more for employer A? Answer ONLY Yes or No.
3b.) In the last 12 months, did you work 1,250 hours or more for employer B? Answer ONLY Yes or No.
