I meant no offence. This OP says he is considering castration but his wife will have unprotected sex with others. It is very strange.

And let's not pretend that all physicians and surgeons will turn up their noses at anything even vaguely resembling unethical. The simple fact is that for the right amount of money, you can find a qualified, still-certified surgeon to perform just about anything you can think of (and, indeed, some things you'd never dream about).

Of course, this is a legal forum and it's not about emotion but legalities ... right? Riiiiiight.
