Clothes really are such a petty issue, IMO. Just stop sending extras. If you feel you absolutely must, send a few pairs of clean underwear and socks with them (and teach them how to rinse same out by hand at the end of the day!).
When my kids were going to their Dad's frequently, he had clothes at his house for them. Both because it was easier and because he has different standards than I do - and I really had no intention of buying them clothes that they'd never wear here. Now that they go less frequently, I have them pack a few changes of (non-objectionable) clothes - it makes no sense for Dad to buy them something that they'll wear once. If he does buy them clothes, they usually come home (with his blessing) as they'll otherwise go to waste.
You don't say what the medical concern is with the diet or the age of the child, so I'll reserve comment on that issue.
